First and foremost, I would like to thank the entire Benefit Administration/ Consulting staff of Menke & Associates, Inc. for their invaluable assistance in developing the sample forms packages contained in this book. These sample forms packages have been developed over the past thirty years by our Benefit/Administration Consulting staff. Without their help and assistance, this book would not have been possible.

Donna Blair, Debby Fulton, Sandy O’Sullivan and Karen Johansen were especially helpful in suggesting the need for additional forms packages to cover different distribution options and alternatives.

Second, I would like to thank Catherine Brandt of ImageXprint.com, who first suggested the idea of converting this book into CD-ROM format so that the forms could be readily downloaded by users. In addition, Cathy provided the technical support to produce the CD-ROM, as well as all of the graphics. As always, the graphics that Cathy provided are excellent.

Third, I would like to thank Lita Romero who did all of the word processing of the forms. More than with most books, this book required an endless number of revisions and an extreme attention to detail. Lita sailed through all of these revisions, while also fulfilling her regular duties, without losing any of her usual optimism and cheeriness.

I would also like to thank Leslie Lampitoc, our in-house Information Systems Director, for her invaluable assistance in converting all of the word files into PDF format and html format. Again, without her help and assistance, this CD-ROM would not have been possible.

Last, but not least, I would especially like to thank Anne M. Johnson, who proofread all of the forms. Anne is one of our Benefit Consultants who is both a gifted administrator and a gifted proofreader. Without her combination of administration knowledge and proofreading skills, this book would not have lived up to its potential.
